Counseling Policies & Client Agreement
Welcome to Healthy Hive! We look forward to working with you to resolve your family’s nutrition challenges and bring joy back to mealtimes. The following guidelines have been established to facilitate our work together. Healthy Hive Family Nutrition, LLC (“Healthy Hive”) provides nutrition services from registered dietitians. RDs are trained professionals in the field of nutrition, however, they are not physicians trained to diagnose and treat medical problems. Any information or guidance given during our counseling session is not intended to replace a medical diagnosis or advice from a medical provider. As a client, you agree to keep Healthy Hive up-to-date on any pertinent health or medical changes that might arise during our time working together.
Healthy Hive provides evidence-based solutions to family nutrition challenges. Oftentimes, these nutrition challenges take time and patience in order to see results. The success your family achieves in this nutrition program depends on your willingness to implement change in a consistent manner, practice patience, and follow up with Healthy Hive as needed for additional support. Healthy Hive makes no claims, guarantees, or warranties regarding the results your family should obtain under its direction.
Your privacy is very important, and all sessions will be held in strict confidence. If we feel that it would be beneficial, we may request a release of information to obtain medical records from your child’s provider. You are not required to sign this release if it is requested.
Payment for counseling services is due at the time of booking. A receipt will be provided for services rendered. Healthy Hive does not accept insurance at this time, but can provide you with a Superbill that you can submit to your insurance company if you have out-of-network coverage. A Superbill is a detailed receipt of services provided by a healthcare practitioner. Submitting a Superbill may reduce your out-of-pocket expense for nutrition services. However, not all insurance companies cover nutrition services. It is your responsibility to determine what your insurance may or may not cover and to make decisions accordingly. Payment for services is still due at the time of booking, and any reimbursement from insurance would go to you directly as Healthy Hive would have already been paid for services provided.
If you would like to contact your insurance company to determine what is covered, we recommend asking the following questions:
Do you reimburse for the following CPT codes in a telehealth setting: 97802, 97803, 97804. If so, how much do you cover? Please be very clear that it is a telehealth service as this may or may not affect your coverage.
Do you reimburse for ICD-10 code Z71.3 (Dietary counseling and surveillance), or is a medical diagnosis required from a physician? If a medical diagnosis is required, we can provide you with a referral form to submit to your child’s medical provider.
Cancellation and Refund Policy
We are busy working mamas, and our time is valuable just like yours! We ask that you contact us as soon as possible if you need to cancel or reschedule your appointment. You can e-mail us at firstname.lastname@example.org to request a cancellation/reschedule. Appointments that are cancelled at least 48 hours prior are eligible for a full refund or may be rescheduled for a later date. Appointments that are cancelled less than 48 hours in advance will not be refunded.
We understand that life can be hectic, and sometimes it’s challenging to be right on time to an appointment. If you are running late, we will hold your appointment for 15 minutes from the scheduled start time. If you arrive to our session within this time frame, we will gladly honor your appointment, but your session will not be extended past its scheduled time. After 15 minutes, your appointment will be forfeited, and you will not be refunded. We understand that technical issues may arise when conducting sessions virtually. Please try to arrive a few minutes early to our virtual appointment to ensure that any technical issues are resolved prior to our meeting so that we may have as much uninterrupted time with you as possible during our session.
